What to Bring to Your Child's Appointment

• Name, address, and phone number of referring physicians
• Insurance card
• A form of personal identification (e.g. driver's license)
• A list of current prescription or over-the-counter medications your child is taking, including dose and frequency
• Previous tests and evaluations

Payment and Insurance

We request payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan. If you have any questions regarding patient billing concerns, please call us at 845.627.2950. Questions regarding your coverage and benefits should be directed to your employer's benefits office or insurance company.
